1. Patient Coordinators that take prescription orders and carry out mini triage over the phone
2. Specialist Nurse and nurse prescribers that are able to immediately write prescriptions as well as visit patients in their home or at a clinic

Centralising these critical elements of the patient pathway in a specialist centre of excellence has created a clearer integrated patient pathway (from acute to community).

How can Connect support stoma patient care and prescribing?
